ACCESS PLAN:

What is ACCESS?  ACCESS is an acronym summarizing the competencies all students should have if they have been through a data-driven, comprehensive, developmental school counseling program K-12 based on the American School Counselor Association (ASCA) National Model Framework/ASCA Standards with Added Emphasis on College and Cultural Readiness. (Chen-Hayes, 2010).

Here you will find the ACCESS and Accomplishments plan that you will complete for your school counselors to get a better understanding of your needs and interests.


ACCESS PLAN:
A questionairre to be completed by students to assess their academic, career, and personal/social growth. 

1.  What has been your greatest academic accomplishment?

2.  What skills have you attained thus far that will carry you throughout college and career options?

3.  How can the responsibilities/expectations you have in middle school be applied as you move on to HS, college, and at work and home?

4.  If you were the principal of the HS or college you want to attend, what would be your selection process/criteria for students and why?

5.  List three things that you need to know when preparing for an interview.

6.  What subjects are you interested in now that may impact your decision in choosing a high school or career option?    

7.  List three characteristics that make a good friend and why?

8.  If someone you know is being bullied, what could be some ways to help them?

9.  What is abuse?  List the different types you know and provide an example for each.

10.  What extracurricular activities are you involved in that will help you to build the skills you need
for the future?

What are some challenges that you may face during your transition from high school to college?

List three aspects of the college and career admission process and why are they important.

A questionairre to be completed by students to assess their personal interests, ambitions, and accomplishments.

3.  What are your three top potential college majors of interest?

4.  What type of college/career experience would be the “best fit” for you?

5.  List any honors/awards you have received?

6.  What extra-curricular activities do you participate in?

7.  What have your involvements been, if any, with sports/athletics?

8.  What leadership roles have you held and/or currently hold?

9.  What adults would be best to write a recommendation letter for you for college/career?

10.  What would you want your teachers/school counselors to say in a letter of recommendation about you?
